Important Forms for Parents

These forms need to be completed and turned into the church office before any student, even a friend, can go to summer camp, a mission trip, or any event outside of Pikeville or involves traveling.

These forms are on file for only the calendar year they were completed in.  If the last form was completed in 2008, then a new form needs to be completed and filled out for 2009.

Each form is required to be notarized, and a notary is available at the church office to freely witness these forms.

Parental Consent and Medical Release Form pdf:  Authorizes care of student in case of medical necessity and provides student medical information and gives student your permission to go on a trip or participate in an event.
